Changes Made to Field Trip Request Forms

There have been a couple of additions made to the field trip request forms. We have added two signature lines-- Cafeteria Manager /School Bookkeeper. Please make sure that both of these individuals have signed off on the form prior to giving it to Mrs. Choplin for approval. The changes were made to make sure that the cafeteria folks are always aware of when a classes are away on a field trips or when they may need bag lunches. There have been several times when due to hectic schedules the cafeteria was not notified of field trips and they over prepared food for the day. Thank you for your help with this matter.
